Essential heart Community Assessor Full Time - Poole Branch

Due to the award of two large local tenders, we have an extremely rare opportunity for a Full Time Community Assessor.

We have worked hard to build our reputation within the care industry and we are looking for a hard-working individual to join our Assessment Team. As a Community Assessor you will be meeting new service users and assessing their care needs, as well as re-assessing our existing service users in line with detailed company and industry requirements.

You will also be expected to work on a weekly rota, alongside your team members, delivering completed assessed files and assisting with MAR charts and medication queries. The Community Assessor will also be required to assist with hands on care in the community, as required.

The successful applicant should have at least five years’ experience in a care environment, be a car driver, possess excellent communication, literacy and computer skills, as well as a team player who wants to make a difference. This is an extremely demanding role and we are looking for a dynamic team player who can work under pressure to ensure that we adhere to regulations, maintain our excellent reputation and most importantly ensure that accurate information is obtained from our new and existing Service Users in order that our domiciliary care department can run effectively.

Essential heart On Call Co-Ordinator

We are looking for a weekend and evening on call co-ordinator to work alongside our current on call team from January 2018.

Our on-call co-ordinator will provide advice and support to our staff and service users. Our on-call system is extremely robust with support from additional staff back up. This position is mainly home based, however there is requirement for two hours in the office (alt Saturday and every Sunday morning from 0830 – 1030 hours).

The remainder of the on call is worked from home. A phone is provided, and the successful applicant will have experience of working within the care industry, excellent communication skills, the ability to work in a fast paced environment and experience of working under their own initiative and as part of the team.

The salary is extremely competitive for the right applicant and full training and support will be given.
